H1B for Non-US Licensed? Possible?

I have more than 5 years experience as Physical Therapist. I have all the documents from Visa Screen, English Exam, and Credentialing done. The only thing that I don't posses now is the US License. I have been approved by the USCIS in California for H1B but have been turned down by the US Consulate from my home country because of lack of licensure. I was put on hold and was given 221g letter and waited for almost half a year now. What has gone wrong with the application that I have been told that there has been some mistake with me being approved by USCIS for H1B. If anyone knows about this please let me know.
