may separate application form kasi para sa mga aplikante para sa NY Licensure, here's the link ... www.fccpt.org/forapplicants.html#app4
pwede ka na agad mag send ng application form sa NYSED, here's the link ...http://www.op.nysed.gov/pt.htm
but it is important to note that your application to NYSED is dependent on FCCPT, meaning hindi magsisimula yung evaluation ng NYSED ng mga documents mo unless they are verified by FCCPT, it is the FCCPT office who will forward your documents to NYSED, and by then, you should receive an email from FCCPT informing you about it.
in my case, it took FCCPT 3 months, nasa NYSED na yung papers ko and i'm just waiting for the result.
