I am still in the Philippines and already have an eligibility to take the exam for Iowa. With the decrease in the number of quota for H1b visa I am thinking of applying for a tourist visa to take the exam in Guam. Would this be possible? What do I need to present to the consul? What are the steps that I should do?

No, it is not mandatory to take the exam in GUAM. Usually, Filipino PTs prefer to take exam in GUam because the expenses incurred is cheaper and the probability of Visit Visa approval is higher than in other jurisdictions.
