To make full use of the attachment feature of this site,
I thought of posting here a sample copy of the clinical summary report required for ICHP evaluation,
it was the one prepared by "stockypinoy" in thefilipinotherapist yahoo group.
I wonder how is he now?

Good day. I am new member on this forum. I really appreciate this site and moreover it is a great help to every Filipino PT who is hoping to work in the U.S someday. And I was one of them. Thank you so much and God bless us all.
